I recently put a charge of $4000 on my 0% APR credit card. I have thus far paid it down to $2000. The 0% rate runs out at the start of June 2008. If my balance in June isn’t entirely paid off let’s say it’s at $500 will my 9.99% interest rate be applied to the $500 remaining balance or the starting $4000 balance

It depends on the terms of the 0% offer when you accepted your offer. Some credit cards apply the new APR to just the outstanding balance and others apply it to the entire thing and you will see different terms from the same company depending on the promotion.
